Begin with shared information

Share reliable measurements, the actual medication list, and an agreed goal before changing treatment.

Define complementary roles

Assign complementary roles for monitoring, prescribing, education, access support, and referrals within the local scope of practice.

Partner with the patient

Ask what makes the plan difficult, then agree on a feasible change and check understanding through teach-back.

Create a review cycle

Link every measurement to interpretation, an authorized action, and a dated reassessment with explicit escalation responsibility.

Interpret the outcomes evidence

Team-based programs improve blood-pressure outcomes, but the effect depends on implementation and setting; a trial result is not a universal guarantee.

Measure continuity and equity

Track outcomes and unequal gaps, and confirm that the current plan reaches the next responsible clinician during transitions.
