Antibody-mediated injury

Antibodies directed against basement-membrane targets can injure glomeruli and pulmonary capillaries. Renal and pulmonary disease may present together or separately.

Recognize the emergency

Rapid kidney deterioration with glomerular hematuria, pulmonary hemorrhage, or unexplained anemia and respiratory symptoms warrants urgent specialist evaluation.

Confirm rapidly

Use anti-GBM serology, ANCA testing, urine findings, imaging, and biopsy when appropriate. A negative or discordant test requires clinical interpretation.

Coordinate immediate care

Disease-specific treatment, antibody removal, immunosuppression, and organ support require expert coordination. The balance depends on severity and potential reversibility.

Assess prognosis honestly

Kidney function at presentation, biopsy chronicity, dialysis requirement, and pulmonary involvement influence expected benefit. Avoid treating all presentations as equivalent.

Follow both antibodies and organs

Monitor clinical recovery, lung status, kidney trajectory, complications, and overlapping ANCA disease. Laboratory clearance is not the only outcome.
