Antibody-mediated injury
Antibodies directed against basement-membrane targets can injure glomeruli and pulmonary capillaries. Renal and pulmonary disease may present together or separately.
Recognize the emergency
Rapid kidney deterioration with glomerular hematuria, pulmonary hemorrhage, or unexplained anemia and respiratory symptoms warrants urgent specialist evaluation.
Confirm rapidly
Use anti-GBM serology, ANCA testing, urine findings, imaging, and biopsy when appropriate. A negative or discordant test requires clinical interpretation.
Treat strong suspicion urgently
KDIGO advises starting treatment without delay when anti-GBM disease is strongly suspected, even before confirmation. Plasma exchange, glucocorticoids, and cyclophosphamide are the standard disease-directed framework for appropriate patients; arrange urgent nephrology care, pulmonary support, and diagnostic confirmation concurrently.
Use the exception precisely
Dialysis dependence with extensive irreversible biopsy injury and no pulmonary hemorrhage can change the benefit of aggressive therapy. This is a specialist decision based on the combined presentation. Pulmonary hemorrhage and dual ANCA positivity alter the reasoning; dialysis requirement alone is not a universal reason to withhold care.
