Rapid injury has several causes
Rapidly progressive glomerulonephritis is a clinical syndrome of rapidly worsening kidney function with glomerular inflammation. Anti-GBM disease, immune-complex disease, and pauci-immune vasculitis require different diagnostic and therapeutic interpretations.
Recognize the urgent pattern
AKI with hematuria, proteinuria, and active sediment is concerning, especially with pulmonary hemorrhage, systemic inflammation, neuropathy, or ENT disease. Escalate suspected organ-threatening disease promptly rather than waiting for every confirmatory test.
Integrate serology and tissue
Assess ANCA antigen specificity, anti-GBM antibodies, complements, infection, and other targeted studies. PR3 and MPO associations are informative but imperfect. ANCA positivity alone does not establish vasculitis, and some active disease is ANCA-negative.
Treat through a specialist pathway
Induction and maintenance therapy depend on disease type, severity, kidney function, relapse risk, and treatment toxicity. Plasma exchange has selected indications and should not be assumed to benefit every patient with severe ANCA-associated renal disease.
Track recovery and complications
Monitor renal trajectory, urine findings, pulmonary status, infection, blood counts, and treatment adverse effects. Dialysis at presentation does not automatically preclude recovery; chronic damage and the overall clinical course influence prognosis.
Evidence update: avacopan
ADVOCATE, historically cited for avacopan efficacy, was retracted in 2026. Its reported benefit should no longer support treatment claims. Preserve the historical citation with the retraction notice and reassess the remaining evidence with current specialist guidance.
