Ammonia handling can fail

Impaired hepatic clearance, increased production, portosystemic shunting, or inherited metabolic disease can raise ammonia. Severe elevations can contribute to cerebral dysfunction, but interpretation and urgency depend on the clinical syndrome.

Recognize neurologic deterioration

Altered behavior, confusion, somnolence, seizures, or coma require urgent evaluation. Acute liver failure and selected metabolic conditions have risks that differ from chronic cirrhosis with hepatic encephalopathy.

Obtain a reliable measurement

Follow local collection and processing requirements and interpret the result with the examination and disease context. An isolated ammonia value does not fully grade encephalopathy or replace investigation of other causes of altered mental status.

Treat production and the cause

Manage the precipitating condition, support airway and circulation when needed, and use syndrome-specific medical therapy. Coordinate hepatology, intensive care, metabolic expertise, and nephrology when the patient is seriously ill.

Use KRT for selected situations

Dialysis can remove ammonia; modality choice depends on clearance needs, hemodynamics, cerebral risk, and ongoing production. Continuous support may help control rebound, while treatment intensity and timing require specialist judgment.

Avoid a universal threshold

A numeric cutoff from one disease or age group should not dictate dialysis for every patient with liver disease. Evidence quality varies, and ammonia reduction must be considered alongside neurologic and overall clinical outcomes.
