Treat urgent threats while you evaluate

Look for transplant-related deterioration, hypertensive emergency, symptomatic severe hypercalcemia and other acute threats. Reconnect electrolytes, kidney reserve, urine protein and structural disease before choosing a treatment.

Four urgent assessment domains: heart, brain, breathing and perfusion, and kidney/metabolic threats; stabilize and investigate in parallel, then reassess.

Treatment and reassessment guide

Diltiazem inhibits CYP3A and P-glycoprotein, increasing tacrolimus exposure. Tacrolimus toxicity can cause renal vasoconstriction, tremor or headache, impaired potassium excretion, magnesium wasting, hyperglycemia, and hypertension. Other causes of graft dysfunction must still be considered.

The Calcium Connection · Summary-day capstone

Integrate electrolytes, CKD, AKI, stones, myeloma and nephrotic syndrome from AL amyloid. The case reveals the seven-lab calcium pattern in stages, then compares nonalbumin light-chain protein with albumin-heavy glomerular disease.

Hypercalcemia: read the whole pattern

Six common hypercalcemia mechanisms compared across calcium, phosphate, alkaline phosphatase, 1,25-dihydroxyvitamin D, 25-hydroxyvitamin D, PTH and PTHrP, with caveats for kidney dysfunction and normal ALP in myeloma.

Monoclonal proteins: two different kidney patterns

Comparison of light-chain cast nephropathy with predominantly nonalbumin proteinuria and AKI, and renal AL amyloidosis with albumin-heavy nephrotic syndrome; tissue diagnosis and amyloid typing distinguish lesions.
