Treat urgent threats while you evaluate

Look for hyperkalemic cardiac toxicity, hypoxemic pulmonary edema and uremic complications. A stable CKD stage or an isolated office BP value calls for context and follow-up, not reflex emergency treatment.

Four urgent assessment domains: heart, brain, breathing and perfusion, and kidney/metabolic threats; stabilize and investigate in parallel, then reassess.

Lab interpretation guide

Side-by-side comparison: SGLT2 inhibition reduces proximal sodium–glucose reabsorption and increases NaCl delivery to an intact macula-densa NKCC2 sensor. ATP and adenosine signaling can constrict the afferent arteriole and lower glomerular pressure. Loop diuretics block NKCC2 in the thick ascending limb and at the sensor, blunting this feedback and favoring renin release. Net GFR also depends on perfusion, volume, and congestion.

Use the diagram with the teaching text, then apply it to the case findings.

Treatment and reassessment guide

Vomiting, diarrhea, and poor intake can reduce renal perfusion. NSAIDs, ACE inhibitors or ARBs, and diuretics can compound the hemodynamic problem. Illness and fasting during SGLT2 therapy raise ketoacidosis risk. Vomiting may add metabolic alkalosis, while diarrhea may add normal-gap acidosis.

Heart failure: venous congestion impairs sodium excretion. Nephrotic syndrome: albumin loss and renal sodium retention contribute to edema. Cirrhosis: splanchnic vasodilation reduces effective arterial filling and activates sodium and water retention. These processes can overlap.
